REGISTRATION FORMS
ETA maintains an open enrollment policy for new students and volunteers, even though we teach on a quarterly basis. New students and volunteers are accepted at any time. Please print these files off and mail them to ETA once you have completed them. Send us an email to let us know that they are coming, and include your complete mailing address. This should be returned with a check if you are registering for a class or an evaluation, or if you are requesting a scholarship application package. Please also keep a copy of your forms for your own records.
